Medical Release / Permission to Treat

Jesus promises to keep your soul secure, and we promise to help keep you safe! To help us help you in the event of an emergency, all adults and minors participating in any event sponsored by The Woods Baptist Church must complete this form. A new Medical Release / Permission to Treat form must be completed each year by all participants
